Appointments


Please print and fill out the forms pertaining to you below in advance. You will need to bring these forms the day of your scheduled appointment. These forms are important–they help determine the course of your treatment. We have to know about allergies, sensitivity to anesthetics, long-term medications–your whole health picture.

Your initial exam will last approximately one hour. During this first exam we will be gathering information to develop your treatment plan. Here is what you can expect:

  • A very warm and cordial staff.
  • An initial assessment of your needs by a treatment coordinator.
  • A tour of our office.
  • A complete series of Radiographs and Panoramic Radiographs to help detect any decay, bone loss or abnormalities.
  • A complete head, neck and soft tissue exam as part of an oral cancer screening.
  • Periodontal probing to check for gum disease.
  • A thorough evaluation of your teeth to check for decay, bite discrepancies, discolored teeth and missing teeth.
  • A series of digital photographs to assess your smile.
  • A treatment plan and consultation at the following appoinment.
